Requesting Your Medical Records Just Got Easier

Sunshine Community Health Center is now working with HealthMark Group to make sure your medical records are handled quickly and correctly.


How Do You Request Records?

You can send your request online using HealthMark’s Request Manager. Just follow these steps:

  1. Go to: https://requestmanager.healthmark-group.com

  2. Log in or create an account.

  3. Click "Submit Request" from the menu.

  4. Fill out the form to give permission.


Once your request is done, you’ll get a message by email or mail. You’ll be able to download your records when they are ready.
